Job Description

Implement Health Promotion/IEC (Information, Education and Communication) activities with patients and local communities to improve knowledge, awareness and skills on relevant medical topics. Lead the team of Community Health Workers, who are working in the communities to provide health education, basic medical care (for example Malaria), patient follow up (for example for malnourished patients, antenatal care) as well as support the mobile clinic activities.

FUNCTIONAL AND HIERARCHICAL LINES:

Hierarchically & functionally accountable to HP-IEC Supervisor & Medical Activity Manager

ACCOUNTABILITIES:

• Inform patients and communities about DWBI and services it provides.

• According to medical priorities, deliver information to the patients and community on specific health topics

• Implement activities of health education and awareness (sessions in schools, churches, community), under the indications and supervision of the HP Supervisor.

• Under the guide of HP Supervisor, assess the impact of activities (with questions, verifications and behaviors observations), and report problems, success and constraints to him/her.

• Collect all type of information related to specific health and living conditions of the population, in particular to identify the most vulnerable populations to target.

• Report to HP Supervisor: activities, problems met, strengths and constraints.

Context Specific Accountabilities:

• Supervise and monitor Integrated Community Case Management (iCCM) activities and patient follow up activity in Nutrition and Antenatal care (ANC) Programs of community health workers (CHWs) in all mobile clinic locations by organizing regular field visits. (75-80% in the field)

• Participate in new CHW recruitments, and prepare theoretical and on-job training schedules with the support of HP supervisor and medical team supervisors.

• Monitor CHW skills and accomplishment monthly during training period.

• Once CHWs are competent as iCCM workers, monitor the quality of care on 3 monthly basis by working collaboratively with medical and paramedical team.

• Collect qualitative and quantitative community data using community engagement methods.

• Work closely with the data entry operator for accurate entry of the data for the department and crosscheck the data with CHW.

• Summarize monthly CHW reports and submit to HP supervisor.

• Facilitate emergency case referral by CHWs from the community to district hospital by collaboratively working with District Hospital community health worker (DH – CHW).

• Collect and provide the weekly consumption data from the outreach health workers on time.

• Placing a stock request every month for the medial and non-medical supply. Collaborate with the pharmacy technician to provide the CHW with the medical supply.

• Identify barriers and enablers to health care, understand local health knowledge, practices (e.g - nutrition habits), and seek solutions to barriers in dialogue with the community.

• Organize monthly CHW meeting, discuss on achievements and challenges and prepare work next month work plan.

• Support Identification of external health sectors (Mitanins, Angawadies, traditional birth attendences) in all clinic locations and catchment area; update the records regularly (twice a year)

• Conduct regular engagement meetings with beneficiaries in all designated mobile clinic location and share regular update to HP Supervisor, Medical Activity Manager and Project Coordinator.

• Organize and participate in community meeting.

• Participate in creation of any material needed for the activities of information, education and communication

• Other ad hoc tasks assigned by supervisor (Community systematic data collection - active case findings, active survellience in outbreak response, assessments etc..)

REQUIREMENTS:

Education: Essential: recognized nurse degree/diploma

Desirable: Bachelor in Science of Nursing, Education in Social Science

Experiences:

Desirable: at least 1 year experience in nursing, community mobilization, social work or teaching

Languages:

Essential: Hindi and English

Desirable: Gondi

Knowledge:

Essential: basic computer literacy (word, excel)
