Job Description

South 24 PG Network of People Living with HIV/AIDS (SNP+) is a professionally managed committed CBO actively working for community capacity building and issue based networking and advocacy. The primary focus of SNP+ is on the problems of people living with HIV/AIDS and HIV-TB co modality in their struggle to obtain a life of justice and dignity. Environment stabilization, security, empowerment and improving the quality of life of community people living with HIV/AIDS for self-governance, are the most important components of its mission. Gender justice, community health and livelihood, child education and social security are our primary areas of concern.

Educational Qualification:

S/he should be a Post graduate in any discipline of Social Sciences preferably with 1-year experience of TB and HIV/AIDS programme.

Or

Graduate in any discipline of Social Sciences subject with minimum three years

experience TB and HIV/AIDS programme.


Details of Scope of activities—

The main objective is strategic intervention on TB for people living with HIV (PLHIV) availing treatment under 5 ART centres of Kolkata and their close contacts. The actual area of intervention would be 10 UHDS (Alipore, Bagbazar, Haji, Manshatala, Maniktala, MTM, TB, S.B. Road, Tangra & Tollygunge).


Contact Tracing -

? The service provider will collect the active list of PLHIV(People Living with HIV) from 5 ART centers from State AIDS Control Society(SACS) and will trace down and search close contacts of PLHIV infected with TB ,within and outside Kolkata .

Sensitization of PLHIV and their close contacts –

? PLHIV and their close contact will be sensitized on TB and its correlation with HIV & AIDS during home visit or in ART center during OPD hours .

Screening-

? Within 7 days of notification of PLHIV patient with TB infection, his/her close contacts would be screened .

? During home visit 4’s screening will be done and all the symptomatic presumptive cases(family members / close contact of diagnosed PLHIV infected with TB ) and will be linked to concerned DTC for free diagnostic Service.


Free diagnostic service&Sample Transportation


Establish linkage with concerned DTC for free diagnostic service(sputum microscopy, molecular diagnostics, line probe assay, culture & DST, pretreatment evaluation of DR-TB, tests for TB infections as well as any other tests as per NTEP guidelines) in public diagnostic facilities(DMC)all symptomatic presumptive cases .

If required ,service provider will make arrangements to collect and transport sample of children and EP symptomatic presumptive cases to the nearest DMC by the community outreach staff .

Follow up with DTC for diagnosis report and ensure notification of persons diagnosed with TB in NI-Kshay. UDST status, comorbidity status of the patient is also need to be updated in the portal through DTC.

Service provider will also be responsible to facilitate initiation of treatment of person diagnosed with TB among contacts. Refer TB patients for treatment and provide contact details to concerned TU for patient follow-up as needed.


TB Preventive therapy –

Will identify and coordinate with NTEP for initiation of eligible contact on prophylactic medication -Isoniazid Preventive Therapy (IPT) for children below 6 years age or other preventive treatmentas per the prevailing guideline of NTEP.



Free Drug –

Coordinate with DTC to ensure regular provision of FDC for TB positive contacts of PLHIV with TB infection and drugs for preventive TB .



Treatment Follow-up, counselling and adherence monitoring –


Service provider will coordinate between DTC & ART center for treatment and follow up of PLHIV with TB infection and their close contact who have been diagnosed and initiated on treatment.

Close contacts of PLHIV with TB infection will receive counselling and adherence monitoring support time to time.

Counsel the family on importance of treatment completion.

Identify any Adverse Drug Reaction(ADR) and address it immediately.

Support completion of preventive treatment.


Facilitate access to DBT –


Facilitate all TB patients on NPY and its components. Service provider will help patients for bank account opening who do not have any and will ensure updation patient bank account detail in portal through DTC.


NI-Kshay Reporting-


Service provider will ensure real time reporting in NI-Kshay through DTC.

We have been serving the community for the past 17 years and our logo helps to symbolize that we are persistently working to ignite the flame of awareness, rights, equality and fraternity in the human hearts. The network was formed on 21st November of year 2004 and was registered in 2005 under the Societies Registration Act. It was formed with Seven PLHIVs of South 24 Parganas. Five Sub-Divisional Level Networks were also formed on 2010.We tied up with WBSAP&CS AND NACO,CINI, NCPI+, SAATHII, AAWAZ for nutritional support since 2006, Rotary International for Non-formal Education in the year 2015-2016, WBWDU Vocational Training for underprivileged women year 2017-18, CINI (2017 to 2019) for the treatment,
