Sankalp Samaj Sevi Sanstha is not for profit making organization and working since 1994 in Madhya Pradesh. Presently we are working in the 14 districts of Madhya Pradesh and implementing different projects. Our core area of works is HIV/AIDs & Child Rights Protection. This project is supported by Gujarat State Network of people living with HIV and Alliance India. Technical agency of project is NACO and MP State AIDs Control Society, Bhopal.

Job Description:

Sankalp Samaj Sevi Sanstha invites applications for eligible candidate for the post of CLH & EVTHS (Community Liaison for Health & Elimination of Vertical Transmission of HIV and Syphilis) for Gwalior, Morena, Guna & Shivpuri districts, in "CSC Samagra Program".

CLH & EVTHS will lead the field activities of the CSC. They will identify and facilitate registration of PLHIV including those with special needs (Pregnant women's, children, KPs) at the CSC. They will support the PLHIV to access health and non-health services through regular follow up.

Specific roles and responsibilities of CLH & EVTHS

• Develop weekly and monthly action plan with the help of the CSC coordinator focusing on the priority clients.

• Outreach to priority clients to provide information on treatment adherence, OI, nutrition, healthy lifestyle and positive living and provide psychosocial support

• To assess the immediate requirement of clients & provide need-based services.

• Monitor ART adherence using the checklist.

• Support the PLHIV Specially Pregnant Women’s & Children to avail social welfare schemes, entitlements and other benefits from the local authorities

• Immediate identification of difficult situation and timely escalation to higher authority.

• Punching the data in TAB on day to day basis and syncing the same once in a week

Required Qualification/experience/skills/qualities:

• Graduate with 2 years relevant experience in HIV/AIDS field.

• Basic understanding on NACP Program and willing to work among PLHIV groups.

• Knowledge of Hindi & regional language. Writing and speaking skill.

• Preference will be given to local candidate who belongs to PLHIV community.

• Willing to use App-based reporting using gadgets.

Remuneration: 9000/- PM and Travel up to 5000/- PM (As per actual basis)
