Job Description

The objective of the Position: The staff functions as the link between the CSC and the community. Facilitate the linkage of People Living with HIV (PLHIV) to treatment services, particularly those who are listed as priority populations for the CSC program. Conduct follow-up of diverse groups to ensure the provision of comprehensive care. Monitor treatment progress closely and undertake initiatives to promote adherence, retention, and access to social entitlements/welfare schemes. They are responsible for conducting home visits for approximately 20-40 PLHIV per month. A minimum of five ORW/CLH/Health Promoters per CSC, supported by the domestic budget, is recommended although the number can vary depending on the number of priority population covered by the CSC.


Key Responsibilities:

• Visit ART centre to support newly initiated clients on ART to help them understand the benefits of treatment, address misconceptions and equip them with coping skills to manage possible side effects of ART

• Give information regarding the services of CSC at the ART centre and motivate the newly registered clients to avail care and support services

• Support clients in completing all baseline investigations and other mandatory tests from the hospital prior to ART initiation

• Provide peer counselling on treatment adherence, positive living and related topics to clients during their visit to CSC

• Support peer counselling of clients, dispensing of ART, maintaining drug dispensing details etc. in case the CSC is engaged in community-based ART dispensing

• Facilitate support group meetings at CSC and ART centre

• Organize community events with the help of CSC Coordinator/Project Coordinator (e.g., health camps, fairs, festivals)

• Organize and participate in advocacy activities, including rapport building with different stakeholders.

• Ensure systematic tracking of MIS & LFU cases and facilitate their link back to treatment.

• Provide support to the ARTC to verify the address of the PLHIV newly initiated on ART.

• Conduct proactive follow-up of PPWs to ensure treatment adherence, periodic ANC check-up, institutional delivery, and post-natal care towards elimination of Vertical Transmission of HIV & syphilis (EVTHS)

• Follow-up of HIV exposed infants (HEI), administering life-saving ARV prophylaxis drugs, and adhering to the EID test schedule for up to 18 months.

• Facilitate linkage of PLHIV diagnosed HIV positive at the ICTC- to the nearest ART Centre for ensuring rapid initiation of ART.

• Conduct follow-up of sero - discordant couples.

• Follow up with PLHIV who are due for VL testing.

• Engage in follow-up with PLHIV to enhance treatment adherence and retention, targeting adherence.

• Facilitate form filling for social entitlements, collect them from households, and hand them to the CSC Coordinator/Project Coordinator

• Undertake any other project-related tasks assigned by the CSC Coordinator/Project Coordinator
